Arm position for cubital tunnel?

I was diagnosed with cubital tunnel a few years ago and was given an elbow splint that keeps my arm in a bent position. When I searched for information on cubital tunnel, all of the sites I found said that keeping it bent restricts blood flow and puts pressure on the ulnar nerve. They also said that it is best to keep the arm straightened out by using a splint, especially when sleeping. So now I'm confused. The doctor that I saw was a specialist...was he wrong about the splint that keeps my elbow bent?

Sleeping on bent elbows is a common cause of cubital tunnel syndrome (CuTS). The best long-term solution is to retrain your body to sleep with your arms relatively straight. One way is to sleep with a splint on your arm. Another option for side sleepers is to sleep hugging a king-size pillow with your arms resting relatively straight above and below the king-size pillow.

Straight vs. bent: You don't want to sleep with your arms locked straight because this puts pressure on your joints and can lead to other pain and injury. But you also don't want to sleep with your arms bent under your head because this leads to CuTS. So it's best to sleep with your arms bent 30-45 degrees up from straight, with your hands in a handshake position. If you suffer from CuTS, you'll also find this position hurts the least and puts the least amount of strain on your ulnar nerve.

Splint: An inexpensive solution is to wrap a towel around your elbow and secure it with an ace bandage or tape. This allows you to bend your arm to itch your nose, but prevents you from easily sleeping on bent elbows. You can also buy a splint, but many splints wrap around the elbow, which can irritate your ulnar nerve and cause pain. The best splint is custom made by a physical therapist. It runs along the inside of your arm, and is held to your arm with a strap around the forearm and a strap around the bicep. With this type of splint, there is no material around your elbow to irritate your ulnar nerve.

One concern is that you have been living with CuTS for a "few years." Typically a patient should try conservative treatment (such as splints, exercises, pain medication, etc.) for no more than 3-6 months before considering surgery. Studies show the longer you live with ulnar nerve damage, the less likely you will heal completely from treatment or surgery. And if your fingers start to atrophy or claw, then you should seek surgery asap.
