Biomechanics and Orthotics - What Do They Have To Do With Heel Pain? by Marco A Vargas
Heel pain is one of the most frustrating injuries that runners encounter because it is extremely painful and difficult to get rid of. Adding to the frustration is the fact that there are hundreds of treatments and devices available that simply don't work and only end up prolonging the agony. I am a podiatrist specializing in sports medicine and I have a unique approach to this particular condition and how it must be treated.
Although there are many causes of heel pain, the vast majority of patients with heel pain have a condition called plantar fasciitis. Since the feet absorb 2-3 times normal body weight during running, this causes the ligament to develop micro tears at the attachment on the bottom of the heel bone. These tears cause swelling which then causes pain. The pain is usually worse when taking the first step in the morning or after being off of your feet for a while and then standing. Initially, treatment usually consists of active rest, stretching, anti inflammatory medication and physical therapy. In severe cases, anti inflammatory injections may be used to reduce swelling and pain. However, this is where most treatments end which is why the vast majority of patients either never completely get better or experience pain again at some point.
In order to completely and permanently resolve plantar fasciitis, it is vital to address the biomechanics of the foot, or the foot function that led to the pain and swelling in the first place. In the case of plantar fasciitis, most patients tend to have tight achilles tendons and plantar fascia ligaments which cause the foot to overpronate or roll inward. In doing so, the arch flattens out excessively causing the ligament to stretch beyond its capacity and tear. However, not all cases of plantar fasciitis are identical. Every case of plantar fasciitis is different because not everyone has the same amount of tightness in the ligament and tendon which leads to varying degrees of overpronation as a compensatory motion. When you add in other variable factors such as body weight, types of shoes, running surface and running style, you can appreciate why not everyone gets better when they buy shoe inserts in the store. In this sense, seeing glasses are similar to orthotics - sometimes over the counter reading glasses will suffice but glasses made from a prescription after being examined by an opthamologist tend to work best for most people. The same can be said for orthotics - sometimes off the shelf inserts may work, but they act more like pads or cushions for the shoe and provide no correction in the biomechanics. These inserts are known as accomodative devices. Custom orthotics, on the other hand, are made from a prescription with specific materials and degrees of curvature to not only support the foot and provide shock absorption, but to actually correct and improve the biomechanics to maintain optimal foot position while walking or running. In doing so, the plantar fascia ligament does not tear or swell during running which prevents pain.
Therefore, patients that suffer from heel pain must undergo both phases of treatment - pain relief and relapse prevention. Much like it doesn't make sense to buy new tires and not have them aligned, heel pain relief will only be temporary unless the biomechanics of the foot are addressed. That's why it is important to see a sports medicine specialist that is knowledgeable in lower extremity biomechanics to get the most comprehensive treatment available for heel pain.

Are Heel Cups/the heel cushion thing worth it?
Today I went and bought a new pair of TS Creator TMacs and the guy tried to sell me something called "Heel Cups", it's that gel cushion thing that you put in your shoe that's supposed to cushion your heels. I said no because I was in a hurry but is it worth it? it's $12 by the way. I was wondering if it will hurt me or aid me? Because I had a feeling he was trying to gip me.
You should wear a cup if your nuts are in danger of being cracked!
