Should I Worry About Toe Walking? by Marco A Vargas
Toe walker occurs when a child walks on their toes or balls of their feet during ambulation. This is usually a normal phase of walking, and most children stop walking on their toes around 2 years of age.
As young children grow and develop, they typically go through several phases of ambulation. Once infants have built up enough strength and coordination to move their arms and legs, they usually want to use this new ability to move from one place to another. Most children begin by scooting or crawling before they begin to walk upright. Once children begin to walk on their own, they also go through phases in walking. New walkers tend to have their legs wide to provide a steady base. In the begining they tend to walk with their arms stretched out for balance. As children progress in walking, one phase many go through is that of toe walking.
Toe walker occurs when a child walks on their toes or balls of their feet during ambulation. Tow walking is considered a normal phase of walking and many children stop walking on their toes around 2 years old. On occasion even after a child's second birthday the child may continue to walk on their toes. Toe walking is usually formed as a habit, but at times toe walking may be associated with other biomechanical or medical conditions.
One possible reason that a child may be walking on their toes is that their Achilles tendon is too short. This condition, which is termed equinus, is common in adults, but is probably less common in children whose ligaments and tendons tend to be more flexible. Underlying conditions which could lead to equinus include cerebral palsy and other muscular or neurological disorders.
So how are you as a parent to know whether your childs toe walking is normal or a sign of a more serious problem? One thing to look for is to see if there are other muscular or neurological issues, such as problems with stiff muscles or a lack of muscle coordination. These signs may indicate that a check-up with your doctor is in order. When the physician examines the child he or she will evaluate the child's range of motion and perform other basic neurological exam. Treatment will depend on the cause of the condition.
To begin treatment, my physicians may prefer to wait to see if the child simply "out grows" the habit; many times the child will simply "outgrow" the condition. When a child's range of motion is limited there are several options. To stretch the achilles tendon many patients may wear a brace or splint either during the day, night or both. One type of brace used is an AFO (ankle-foot orthosis). Serial casting where the foot is cast with the tendon stretched, and the cast is changed weekly with progressive stretching. Botox therapy is used to paralyze the calf muscles to reduce the opposition of the muscles to stretching the Achilles stretching, usually together with serial casting or splinting. Surgical lengthening of the tendon is seen as a last option after 12-24 months of conservative measures have been tried. The surgery is typically done under full anesthesia but if there are no issues, the child is released the same day. After the surgery, a below-the-knee walking cast is worn for six weeks and then an AFO is worn to protect the tendon for several months. For toe walking which results from more serious neuro-muscular conditions, additional specialists may need to be consulted.
