Hidradenitis Suppurativa?

I'm absolutely sick of it!

I'm in pain all day long, especially under the arms and breasts. I can't move my arms, in fear of pain. It's even affecting my driving, work and home life.
Anyone know how to minimise the pain of the boil type things?

I was on antibiotics and steriods for 3 years, no luck there. I've tried draining them with Sugar/soap mix, tried all the creams, the hot compresses, the medical baths and antibacterial soaps/washes. I even had them lanced by my doctor.

I'm at my wits end! Please help!

Oh my goodness, darlin', you have every reason on earth to be fed up. This is an absolutely miserable skin condition.

I would suggest you see a dermatologist with a lot of experience in treating this condition. Dermatologists usually are up on the latest treatments. S/he may even refer you for laser therapy. Perhaps a good plastic surgeon can help. I don't know if surgical excision of the affected skin will help or not-- you might just develop the condition again on the new skin.

Steroids are iffy. If they're too strong or used too long they can thin the skin and/or give you a rebound effect when you stop using them.

I checked it out in the "Color Atlas and Synopsis of Clinical Dermatology," and there is some good news. It usually subsides in women at menopause, so there is light at the end of the tunnel. Also, a predisposition for the condition is obesity. So, if you are overweight (you don't say whether or not you are), maybe weight loss can help.

Also, there is a list of differential diagnoses. Since you have had it so long, I won't bother with the early ones. But the list of late differential diagnoses that can mimic hidradenitis suppurativa includes lymphogranluoma venereum, donovanosis, scrofuloderma, actinomycosis, and sinus tracts and fistulae associated with ulcerative colitis and regional enteritis.

Your doctor probably has checked you for these and ruled them out, but it can't hurt to ask.

Special tests are recommended for the infectious agents, including Staphylococcus aureus, various forms of strep, Escherichia coli, Proteus mirabilis, and Pseudomonas aeruginosia. They usually do a culture from the drainage and see what grows.

Under treatment, the book notes that the "Role for oral 13-cis-retinoic acid in advanced disease is not established, but it appears to be useful in early disease and when combined with surgical excision of individual lesions."

It also says, "With extensive, chronic disease, complete excision of axilla or involved anogenital area may be required. Excision should extend down to fascia and requires split skin grafting."

So, I don't know if any of this helps. You don't say how old you are, but if it gets better at menopause, maybe you could talk to your doctor about the pros and cons of hysterectomy and the resultant surgical menopause.

Xssd-- What Sarah has is not acne, and the site you suggested doesn't even address hidradenitis. Acne can be a predisposing condition to hidradenitis suppurativa, but the severity of the disease she is suffering from is so much greater than acne you really can't compare the two.
